Get Started with a Nutrition Intake Session!

If you would like to become a new patient of Dr. Danielle Heard, DCN, CNS, LDN and find out more about Artemis in the City's nutrition programs and how our functional nutrition services can help you create a balanced healthier life, please email us to let us know of your interest.

You will need to let us know if you have health insurance or would be self-paying for services. If you have health insurance, the Medical Nutrition Therapy (MNT) or Preventive Nutrition Counseling benefits would need to be checked and confirmed. The list of insurance plans in which we are a provider is listed on the Clinical Nutritionist page. Everyone is required to complete new patient intake forms which are sent to you via our patient EHR system.

If you are self-paying for a nutrition coaching program, you would need to select a New Patient Option (1-4) and then add to the cart and check out through Paypal. You do not need to login to the website and create an account.  

Once all of the new patient forms are completed and also any co-pays for insurance are paid, or if self-paying the new patient nutrition coaching program is pre-paid for, your intake session is booked in Dr. Heard's calendar. Please also upload a copy of any recent bioscreen diagnostic laboratory testing you might have into our patient EHR system. If you would like for us to send you to our diagnostic lab service for a complete bioscreen, please contact us regarding the cost and to schedule the lab work. This lab testing must be prepaid.

To pay for your intake session, we accept cash, checks and credit cards. You can mail your payment to our address which can be provided or you may make the payment through our secure online store through Paypal click here. The payment is not made on this website. You do not need to login to this website or create a profile. You simply add the program you wish to purchase and then select checkout and will be transported to Paypal where you can pay with your selected credit card. Checks should be made payable to Artemis in the City in the amount listed on the new patient option that you select. 

Everyone must complete the new patient intake assessment in order to work with Dr. Heard to help you achieve your health goals. Once we receive your completed new patient forms, documents, diagnostic labs, and any required payments you might have, we will contact you to book a session date that fits with your schedule. The nutrition intake session lasts 120 minutes (2 hours).

Our policy is a 48-hour cancellation notification. If you do not call or contact us 48-hours prior to your appointment time or you do not show for your appointment, the session fee is non-refundable as we have held this space preventing anyone else from booking the time with Dr. Heard. If there is a personal or family emergency, we of course will work with you to make up the session time. Life happens to all of us! This cancellation policy is the general policy that appplies to all scheduled session appointments.
